This is Index, from Toaru Majutsu no Index. Screen grabs from episode (i think) 5, where she enters "Dark Mode". The eyes are from Series 2, when she uses Sheol of Fear (My screengrab wasn't a high enough res to get any detail on Dark Index's eyes )
REVISION 2: Anti Aliasing is less jagged :D
