Though I never personally played it, I heard that Conker's Bad Fur Day, for the N64, was pretty bloody. Not sure about the bloodiest though.

Silent Hill 3's quite the blood bath. Especially when it goes into Otherworld mode.

Oh yeah...The Suffering and The Suffering 2..now those were truly undead bloodbaths......
your character can even get blood stained.... and the more he kills or saves...the more his personality and his transformation get more disturbing or powerful...
