Ninjutsu effect, probably. We've seen lots of ninja that end up looking weird because of their ninjutsu, like the Sound 4, Orochimaru, several members of the Akatsuki and so on.

I forget which one it was, but one of his names was the name of a shark demon I believe. Hence Samehade being a shaving sword rather than a regular slashing or stabbing sword. Then there's of course his "Water Element: Water Shark Jutsu." (Can't remember the Japanese name for the life of me) I'm pretty sure it was Hoshigaki, but I could be wrong.
At least that's how he was designed. Whether he began as a human at all I'll never know. And if he did, it was probably some sort of surgical ninjutsu.

@ mad333: Kisame and Orochimaru had absolutely nothing to do with each other, except that they might have been in Akatsuki at the same time. The manga doesn't even so much as hint at that >.<
I agree with redstorm that this could be some ninjutsu effect, but I'll stop at that. I don't usually like to speculate, so for now I'll just stick with "Kishi wanted to draw him that way" :)

I think he's some sort of an anomaly, like a mutant or something, and was born like that. Either that or jutsu or something.

Well, I can't see any logic about that, I mean lol, if Akatsuki guys would all be as perfect as Itachi and Sasori, then it would not be so spiced up.. you need some ugly,terribly looking monstrous villains like kisame XD in anyhow like a shark..

Orochimaru and kisame haven't had any relations in the past. I'm going to bet that it's a family trait or something.
It's because he uses water techniques. He wanted to become more in tune with his attacks so he used special ninjitsu to turn himself into a half shark half human badass.
