Well, I saw this vector (by loveable-IY) and already had the idea for this punchline in mind. I really like the simplicity of it so far, but it really seems to be lacking something. Any suggestions?

Since it's been Haruhi and Tamaki are already vectored, I think you could have done more to the background.
The background looks too simple and it looks like you didn't put much of an effort into it.
Maybe the background should be a clinic, or the host club room? (I'm not too sure, but I'm sure you can do something about the wall ^^")
Also, why don't you try putting other host club members as well?
It may not work, but I feel like there's too many space left even after Tamaki and Haruhi are there.

I agree with Starrys; perhaps the background needs to be revamped. It would look really cute if you could put them in a clinic setting, or at least draw the white curtain that you see patients sit behind in clinics. It'd be cute if there was a fake doctor's set with other tools around Haruhi; it'd be easier to believe they'd be playing doctor together :P
Just a side note - the word 'actually' is spelled wrong in the subtext. should be a quick fix though ;)
Other than that, the quote is very cute and cheeky. I can't think of a witty way to improve it.

I rather like the simplicity of the wall, but I'm one of those people anyway. I would like it better if the background didn't overlap the characters at all, though, so I think if you cleaned it up just a bit it would be crisper. Also, if they have shadows beneath them, there's no reason for an outer glow. It looks like you're trying to disguise some choppy edges, but if that's not necessary then just get rid of the glow all together.

Oh man, this made me laugh so hard. I love it! And the simple background works well, I think. Anything more complicated might just distract people from the joke. If you ever do feel like editing it, I'd recommend playing around with some different, yet equally simple backgrounds, but I think putting them in an actual setting would be unnecessary.

I really think that the simplicity of this wallpaper works. I love it, and I think that the punchline just makes it amazing. The argyle is pretty neat too. I did notice a spelling mistake with the word "actually" though. I just thought I'd point that out.
